Community Public Health Teams (CPHT) Pilot Project
LA County’s Community Engagement Unit sought to strengthen communication capacity across 10 Community Public Health Team service areas. Through in-person meetings with each CPHT’s Community-Based Organizations, Health Care Providers, and Rising Communities partners, insights were gathered to develop tailored Communication Plans for each service area. The project included 12 localized outreach materials (print and digital, translated into Spanish) designed as editable Canva templates, empowering CPHT teams to personalize messaging for their communities. Deliverables included five capacity-building communications trainings with live Spanish interpretation, four video PSAs showcasing the program, and a Success Story Booklet documenting results from the pilot project.
